Eddy SeniorCare (PACE)
Eligibility & Payment Information

Seniors enrolling in Eddy SeniorCare must be:

• at least 55 years of age
• live in our service area (zip codes: 12008, 12302-09 and 12205 northwest of Route 155)
• have health problems that would qualify them for nursing home level care for 120 or more days
• desire to continue living at home
  

Eddy SeniorCare services are funded through a unique capitated reimbursement arrangement with both Medicare and Medicaid or are available through private pay. This unique way of funding allows Eddy SeniorCare to focus more closely on the care you need, when you need it most.
