Sunnyview is a Regional Resource Development Center for the New York State Nursing Home Transition and Diversion Medicaid Waiver Program, under contract with the New York State Department of Health.

Our services are available in Albany, Columbia, Greene, Rensselaer, Schenectady and Schoharie counties and include:

• independent living skills training
• congregate and home-delivered meals
• assistive/adaptive equipment
• day services
• home visits by medical personnel
• home and community support services
• assistance with housing, moving, furnishings
