Advanced Illness Coordinated Care Program

Dealing with an advanced illness can take its toll on the patient, family and loved ones.

Eddy Visiting Nurse Association/Eddy Home Care offers Advanced Illness Coordinated Care (AICC), a program designed to provide information, support and guidance to patients dealing with an advanced illness. Staff members work in collaboration with the patient's primary care provider and medical team to meet the physical, emotional and spiritual needs of the patient, family and loved ones.

AICC can help patients experience personal growth, family closeness and peace of mind by helping with advance directives, specialized care and treatment, and offering support and compassion to the family and patient.
