Sunnyview Rehabilitation Hospital - Community Health

Sunnyview Rehabilitation Hospital, like all of St. Peter’s Health Partners, is committed to improving the health and wellbeing of our community, not only as a caring community member, but also as a catalyst for change.

As such, we participate in many community partnerships aimed at assessing the current health status of our community and identifying opportunities to make a difference in the health of our citizens with particular attention to those who are poor and vulnerable.

As we have done for many years, we continue to play a major role in the Healthy Capital District Initiative, an organization dedicated to improving the health of the residents of Albany, Rensselaer and Schenectady Counties. Our partners in this endeavor are the local county health departments, other health care providers, insurers and community members.

In June, this group completed the 2013 Community Health Needs Assessment (CHNA), which represents a current profile of health status, wellness, health delivery and public-sourced opinions about health in the Capital Region and identifies significant health issues in the communities we serve.

In Schenectady, where Sunnyview is located, this work was further informed by the activities of the Schenectady Coalition for a Healthy Community whose mission was to develop a plan that accurately reflected the major health issues facing the community, to design goals and objectives to materially improve target indicators over the triennium, and to then work collaboratively and independently to implement that plan. The vision is to have Schenectady become a healthier, safer community for all its citizens enabled by community involvement at each step along the way. These assessments contains both quantitative and qualitative data sources, along with significant stakeholder and public input. This information was used in the development of a Community Health Improvement Plan (CHIP) that will guide Sunnyview, as well as St. Peter’s Health Partners Community Benefit programming and activities, for the next three years. It will also help us track our progress.

The CHNA and CHIP together form the Sunnyview's Community Service Plan. This document should not be viewed as a static document but as a dynamic roadmap that will improve the health and well-being of residents in the Capital Region.
